Full Job Description

Merton Domiciliary Therapies and Merton Holistic assessment and Rapid intervention Team is a Multidisciplinary Team providing high quality rehabilitation to Merton Population. The post holder will support adults living with frailty and complex health needs to optimise their independence, health, and wellbeing, and prevent admission into hospital or premature move to a care home. We are strongly committed to the assessment and rehabilitation of patients who are at living within the Merton community. The post holder has a key role within the team; working with patients, service users and their carers in providing highly specialist occupational therapy (OT) assessment and treatment plans to assist individuals to maintain or regain their mobility, independence and wellbeing within their own homes, setting goals and reviewing outcomes in order to reduce the need for long-term support. The post holder will provide local clinical leadership and advice for the OTs and MDT in domiciliary therapies and be responsible for the education and supervision of junior occupational therapy staff, rehabilitation assistants and OT students. We have supportive culture that fosters mutual respect and the post holder will have a key role in further developing and maintaining that culture.Main duties of the job Providing a highly specialist occupational therapy (OT) service as part of a multi-disciplinary team, to patients in their own homes and within a clinic setting. To provide clinically effective, person centred, and evidence based occupational therapy assessment and intervention to patients with complex physical health needs To assist the Clinical Lead in the planning and delivery of occupational therapy services to the patients, their family/carers, students, Apprentices complying with RCOT professional and service standards of practice. To provide support and supervision for junior staff To assist in the management, development and research of occupational therapy services, including service reviews. To hold responsibility for defined projects. To provide support and training to all staff with regard to occupational therapy interventions. To comply with the College of Occupational Therapists Code of Ethics and Professional Conduct To deputise and lead the service as and when needed in absence of Clinical lead To jointly lead clinical triage with other senior therapists and clinical Lead in the team. To be responsible for Standard Operating processes in relation to policies and processes that are needed for smooth functioning of the team. To work flexibly and travel to multiple sites as required.
