Full Job Description

As a Highly Specialist Older Adult Clinical Dementia Nurse the post holder will be accountable for their own professional actions and work within professional codes of conduct. The post holder will work within the four pillars of clinical specialism practice - to include:

  • Clinical (within scope of role)
  • Leadership and Collaborative Working
  • Education and Learning
  • Research and Evidence Based Practice
  • This post will have a heavy clinical emphasis, with a strong requirement to educate others, and incorporate evidence-based practice. The post holder will be a senior figure in the Memory Assessment Service, taking responsibility for access to diagnosis of complex patients and associated prescribing, effective use of nursing time within services, collaborative working with stakeholders and clinical supervision across Gwent OAMH services. The post holder will act as a specialist resource for people living with a diagnosis of dementia at the early stages, and work with other teams to affect the best possible person-centred and holistic care for both patients and carers. The role will also be broad and involve diverse and varied aspects of clinical care and service development The Highly Specialist Nurse - within this post - is required to hold a non-medical prescribing qualification - linked to prescribing within Mental health., We are looking for RMN's who will take on a clinical leadership role in our supportive and compassionate teams. The roles will develop the assessment, diagnosis and treatment pathways to ensure greater efficiency and high-quality patient, and carer experience through the journey of dementia. These are new roles and will involve a degree of autonomy on how the clinician works within the team and across pan-Gwent services., The developmental role will be paid under the annex 21 of agenda for change which is 70% of the Maximum Band 7 scale, until competencies are met. If competencies are not met the capability process will be instigated to explore options moving forwards.

    The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
