Full Job Description

We are looking for a motivated and dynamic pharmacist to join thehaemoglobinopathy service, andcontribute to thedevelopment and implementation of joined-up hospital and community protocols fordrug therapies. The remit will include patients living with Sickle Cell Disease, Thalassaemia and rare anaemiaswithin North East London.You will work with an expert multidisciplinary team, to improve patient awareness and adherence to medication as well as enhanced access to medication. The remit will include drug therapies such as hydroxycarbamide, iron chelation and analgesic medications-including responsible management of opioid therapy. There will also be an opportunity to develop pathways and protocols to enhance access to new therapies.

The post willhelp to provide a comprehensive clinical pharmacy service to patients, the multidisciplinary team and provide highly specialist support to the wider pharmacy teams across NEL. You will also help to develop clinical guidelines and act as a key member of the MDT.

You will assist the service in thedevelopment of effective and appropriate community services within North East London including

· Mapping of the current pathways for provision of therapies and therapy monitoring in the hospital community settings across NEL.

· Help shape how services supporting patients are delivered and accessed in the community.

· Support the development of effective pathways of community prescribing and medication monitoring and compliance within community settings relating to sickle cell and thalassemia.

The post holder will participate in education and training of Pharmacy staff at Bart's Health and will assist in the provision and development of clinical pharmacy services within the Network.
