Highly Specialist Pharmacist- Homecare and Medicines Optimisation

· To lead the homecare and medicines optimisation pharmacy team, to ensure high cost drugs are managed safely and effectively.
· To support specialities and areas that use high cost medicines, working in partnership with commissioners to ensure the most cost effective use of these medicines in line with national and local guidance.
· Lead and manage the homecare pharmacy team, responsible for setting up new services and reviewing homecare provider performance and ensure patients are receiving safe and effective care.
+ Provide clinical support as a senior Pharmacist to the dispensary team and other members of the pharmacy team as required.
