Full Job Description
We are looking for a creative, pragmatic, and organised person who enjoys working with people in the community. This successful candidate will play a crucial role in our community speech and language therapy team, taking a lead on our service delivery to care homes across Sheffield. This post is Band 7, 30 hours (0.80 wte) per week, and is a permanent role. This post provides speech and language therapy as part of the SLT Adult Community Team which provides an SLT service for people living in care homes and their own homes. You will be supporting clients with communication and/or swallowing difficulties, often with multiple pathologies. Working with, and via carers is an integral part of our management approach. The successful candidate would be joining the team during an ongoing project that has a strong focus on collaborative training for care home staff in regards to nutrition and hydration. You would have the significant role of delivering this project, working alongside the team leader and team members to implement and embed practices into everyday working.,
- To provide highly specialist Speech and Language assessment, therapy and support to a highly complex caseload of adults referred to the Adult Community Team.
- To provide highly specialist Speech and Language Therapy input as part of a team.
- The post holder will specialise in the management of patients with highly complex dysphagia and communication impairments.
- To provide clinical leadership for the care home service delivery within the Adult Community Speech & Language Therapy Team
You will have extensive experience of working in the primary and community settings and in particular working effectively with other AHPs, nursing and care staff roles to provide the best patient centred care. Our team has a strong networking ethos. There are strong links with the hospital SLT service and other community services.
The SLT service across both the acute and community teams in STH has approximately 60 staff (49 therapists, 7 assistants and 4 secretarial staff). The SLT teams are highly motivated to provide a quality service to their patients, they are research active and embrace innovation. The service has established processes for supervision and supports personal and professional development. There is a strong clinical support network in Sheffield. All therapists have regular clinical supervision. There are monthly clinical meetings and access to clinical lead specialists including AAC, Older Adults, Voice and Dysphagia.
