Chelsea and Westminster HIV services are looking for an Innovation and Improvement Fellow.
The role has been designed in collaboration with ViiV Healthcare, and marks the second collaboration between CWHFT and ViiV. It follows the successful development of a digital health solution, the Klick app, which underpins the management of stable patients with HIV (PWH) with around 7,000 registered users.
This new project management role will help optimise care pathways for stable PWH to improve patient experience and to realise efficiencies in resource management and capacity within CWHFT., · Review and map current service models to fully understand the patient pathway
· Seek health care professional feedback on current inefficiencies and suggested improvements
· Gain patient feedback to understand preferences, unmet need and establish baseline satisfaction
· Design and plan and deliver quality improvement programmes, ensuring clear aims and measures for improvement identified
· Identify and document benefits that will be realised during and after change implementation
· Conduct analysis throughout project delivery and after project completion to ensure learning is taken forward, and benefits realisation is achieved
· Ensure that robust project control is in place and maintained including project documentation, scheduling tasks and setting milestones, liaising with colleagues ensuring that timescales are met, and project objectives and benefits are delivered
· Write progress reports and formal presentations for submission to a variety of groups and other key stakeholders as required
· Develop effective working relationships with key external stakeholders where appropriate
· Nurture a culture of quality improvement through encouraging and empowering individuals and teams to focus on improving patient care
· Constructively challenge behaviours that hinder change or do not support the strategy and values of the organisation
· Engage with clinicians, management, and other stakeholders to deliver a single and consistent approach throughout the project life cycle

This exciting new role presents a unique opportunity to join Chelsea & Westminster Hospital NHS Foundation Trust (CWHFT) to support our world renowned HIV Service within the HIV, Sexual & Gender Health, and Dermatology directorate. CWHFT is the largest provider of HIV care in Europe and cares for over 12,000 people with HIV (PWH) across multiple sites in West and Central London.
The role has been designed in collaboration with ViiV Healthcare, and marks the second collaboration between CWHFT and ViiV. It follows the successful development of a digital health solution, the Klick app, which underpins the management of stable PWH with around 7,000 registered users.
This new project management role will help optimise care pathways for stable PWH to improve patient experience and to realise efficiencies in resource management and capacity within CWHFT.
Working across the directorate, the post holder will utilise project management resources, digital capacities, clinical insight, and patient collaboration to review clinical pathways to afford more time to address the specific clinical needs of key PWH populations. There will be two key focus areas in year one; remote care optimisation for stable PWH and implementation of long-acting injectable antiretroviral therapy (ART). An additional associated work stream; care optimisation for people as they age with HIV, will be prioritised in year two of the project.
