Full Job Description

Chelsea and Westminster HIV services are looking for an Innovation and Improvement Fellow. The role has been designed in collaboration with ViiV Healthcare, and marks the second collaboration between CWHFT and ViiV. It follows the successful development of a digital health solution, the Klick app, which underpins the management of stable PWH with around 7,000 registered users. This new project management role will help optimise care pathways for stable PWH to improve patient experience and to realise efficiencies in resource management and capacity within CWHFT. We welcome a dedicated clinician who has proven interest in quality improvement, multi-disciplinary learning, and clinical leadership. This prestigious opportunity is highly recommended for aspiring clinical leaders. The role is available to applicants from medical, nursing or allied healthcare professions with a clinical background in HIV. Previous project management experience and delivery is highly desirable. Working across the directorate, the post holder will utilise project management resources, digital capacities, clinical insight, and patient collaboration to review clinical pathways to afford more time to address the specific clinical needs of key PWH populations. There will be two key focus areas in year one; remote care optimisation for stable PWH and implementation of long-acting injectable antiretroviral therapy (ART). An additional associated work stream; care optimisation for people as they age with HIV, will be prioritised in year two of the project.,

  • Review and map current service models to fully understand the patient pathway.
  • Seek health care professional feedback on current inefficiencies and suggested improvements.
  • Engage patients via a patient survey to understand preferences, unmet need and establish baseline satisfaction.
  • Quantify any challenges, opportunities and solutions to understand all mechanisms and touch points.
  • Propose pathways for optimised clinical & non-clinical workflows e.g. integration with Klick, other remote solutions and model potential impact of change.
  • Ensure proposed solutions effectively address inequalities defined during diagnosis phase.
  • Evaluate projects vs defined success criteria.
  • Ensure robust clinical and patient outcome measures are identified and captured
  • Lead on implementation of agreed solutions during pilot phase
  • Commensurate with experience, provide and quality improvement leadership for agreed projects
  • Provide monthly written project status reports on progress, including risks and escalation of issues to Steering Group
  • Production of final project report and publication where applicable.
  • Engagement with Innovation Team to amplify any additional opportunities aligned to project work streams.
  • Monthly Pharmacovigilance reconciliations as per ViiV requirement.
