We can provide great leadership, service development and clinical experiences working with a wonderful team of Rehabilitation assistants, Occupational Therapists, Nurses and Pharmacy Technicians This post will provide great opportunities for varied working across the Home 1st Therapy pathways. We provide therapy assessments and interventions for community admission avoidance, virtual wards and intermediate care needs, discharge support to the local acute hospital, inpatient rehabilitation to the Seacole Virtual Community Hospital and Pulmonary rehabilitation., Home 1st is a multi-disciplinary integrated service encompassing health and social care the focus of which is to work collaboratively with other local providers for the purpose of admission avoidance and supported discharge pathways for the residents of Milton Keynes This role will provide leadership within the Home 1st Therapy team and will: o Provide professional leadership and clinical supervision to the Home 1st Therapists. o Coordinate interdisciplinary assessments to prevent avoidable Hospital admission and re admission, in a variety of settings. o Work collaboratively with MKUHFT and MKCC and the 3rd sector to ensure the right pathway for the patients o Communicate effectively and work collaboratively with the other leads within Home 1st regarding patients to ensure their needs are met. o Provide advanced Therapy assessments and treatment for people using the service. o Contribute to service development o Work with the Professional leads for Practice Development and therapies in CNWL (Physio and OT) to ensure professional development of therapy staff.

Home1st is based in Bletchley and the modern office is a central hub for multi-disciplinary working. You will be provided with ICT laptop, mobile phone, uniform and have mileage expenses paid for.
