Full Job Description

Skills in home tube feeding and be part of a team striving to improve health, quality of life and care experience for patients, then look no further. You would be part of our supportive dietetic team in NHFT. This post will help manage the deliveries for tube feeding items, be a first point of contact for patient queries and support the HEF dietitian teams with their caseloads., The role includes:

- Supporting and assisting the HEF team in the management of patients using home tube feeding. This includes elements of clinical and administrative work.

- Use of electronic systems (support will be provided).

- Understanding tube feeding equipment to support team in administrative processes for tube changes, ordering of equipment, troubleshooting and replacement of equipment.

- To assist in the monitoring of equipment usage.

- Collecting relevant patient-related information.

- Contributing to the safe and seamless discharge of babies, children and adults with feeding tubes from hospital to the community setting.

- Maintain effective communication between teams.

- To report any concerns and queries about patients to the dietitians in a timely manner.

- The post holder will need to be able to work under their own initiative using protocols and guidance to support dietitians and ensure the effective implementation of dietetic treatment for patients.

- To monitor patients on enteral feeds in multiple environments e.g. special schools, nursing and residential homes by home visits, outpatient appoints and telephone reviews. You may be acting as a lone worker e.g. completing weight checks.

- To keep electronic records of all patient contacts.

- The post holder will be involved in supporting training programs, auditing and team administration.
