Full Job Description
We are seeking an experienced manager to support the operational management of our Home First teams in Greenwich., Reporting to the Adult Community Physical Health Service Manager for Home First (Greenwich), the post holder will be responsible for the management, leadership and supervision of the teams constituting the Home First services. This is an integrated role working across both health and social care and the teams have multiple pathways to prevent hospital admission and promote early hospital discharge, with the Joint Emergency Team and Community Rehabilitation and short term assessment teams being central to this approach. The services utilise remote monitoring technology and we are currently building an assistive technology enabled care service with The Royal Borough of Greenwich.
Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. We manage hospital sites including Queen Mary's Hospital, Sidcup and Memorial Hospital, Woolwich, as well as the Bracton Centre, our medium secure unit for people with mental health needs.
