Full Job Description
Are you interested in working with people in a mental health crisis and need home treatment? We are looking for a new Band 6 to join our team (Rochdale Home Treatment Team). If you are an experienced Band 5 ready to make the step in your development, then we would love to hear from you too.
You will work within the home treatment function delivering support to patients and their families using evidence based, profession specific, and general mental health knowledge providing client-centered, recovery-orientated principles and practice for individuals, families and carers.
We provide an alternative to inpatient admission treating patients in their home environment who are presenting in a crisis with significant distress. You will be expected to have a means of visiting people in their own homes (when safe to do so) across the Heywood, Middleton, and Rochdale area.
We are looking for a qualified practitioner- a Nurse, Social Worker, or Occupational Therapist. You will need to have experience of community mental health work and/or inpatient mental health services and to be passionate about this area of work. We are more than willing to support people ready for their next stage of their career.
Main duties of the job
The post holder will review patients who are seeking self-discharge from the wards, providing a safer discharge and any Rochdale HTT patients that present at A&E.
They will also complete home visits if risk assessed safe to do so, providing a greater flexibility for supervised medication regimes for patients.
To support patients with telephone calls if required.
To be involved in the ongoing development of the service.
To undertake accurate psychosocial / risk assessments and act as co-worker for service users who are supported by the Home Treatment Team who present with acute mental health problems or psychological distress. To work in close liaison with other members of the multidisciplinary team, including care coordinators, to implement treatment / management plans in order to facilitate Home Treatment.
To undertake the role of the gatekeeper for all people requiring admission to inpatient services, including those requiring assessment under the MHA and to provide an alternative to hospital admission through Home Treatment interventions where appropriate.
It is expected that you will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act and community care legislation.
