Full Job Description

The Homefirst Team is part of the integrated community team west Cornwall comprising of registered and non-registered nursing, therapy and administrative staff working collaboratively with partner organisations to support the intermediate care needs of our local population., You will need to be able to a triage, assess and provide evidence based treatment plans for adults within the catchment area, either in their own homes or nursing and care homes.

To work alongside the wider multidisciplinary teams within the community, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services.

This position is open to Physiotherapists, Occupational Therapists and Nurse Assessors with experience of working in the community and wish to develop their assessment, care planning and reablement skills whilst being part of a dynamic, integrated, multiagency, multidisciplinary team.

You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs and set realistic goals. You will need excellent organisational skills along with a wide knowledge base and experience of working in older person's services.

We will offer a working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system.

We are based in Camborne and cover theWest Cornwall area of Camborne / Redruth and surrounding areas, a valid UK driving license and access to a vehicle for work use is essential.

This is a full-time post, days are planned over a 7 day rota, this post does involve regular weekend working and the successful candidate must be flexible with their working days. We cover 8am-6pm.
