Full Job Description
Would you like to work for an innovation team. The Hospice at Home Team cares for patients in the last 2 - 4 weeks of life at home within Bournemouth and Christchurch . We are looking to expand our current Hospice at Home Service to include the Poole conurbation. If you are enthusiastic, compassionate, motivated Health Care Assistant looking to be involved in providing excellent end of life care why not come and join us. This is an amazing opportunity to be a part of a reputable service and make a difference to so many families., To be able to work independently and assist in carrying a caseload under the supervision of senior nurse, tasks according to specialist nursing need/assessment at end-of-life.
To provide essential holistic, high-quality nursing care as directed by individualised care plans in the patient's own environment, respecting their backgrounds and beliefs.
To provide hands-on practical care and emotional support to patients and their families/carers, reporting any changes to the senior nurse.
To prepare the patient's environment for care at home, maintaining their dignity and privacy and ensure and maintain a safe working environment for patients, families, staff, and colleagues working within Lone Worker Policy.
Recognise the Equality and Diversity of patients, relatives, and MDT groups, by supporting non-discriminatory practice & acknowledging the patient's personal culture, beliefs and identity.
The Hospice at Home service is a 24 hour/7 days a week service, based at the Macmillan Unit, Christchurch Hospital, predominantly visiting patients and families in their own homes to provide end of life care. Candidates will need to be able to drive and have a current UK licence, have experience of the care environment with the confidence to work independently and be flexible with working pattern to include nights. There will be an induction for any training requirements on starting.
