Full Job Description

The successful candidate will be part of the Integrated Community Team and will work closely with the multi-disciplinary team and other care providers, working as a community nurse under the direction of the specialist nurses. You will provide advice and support to patients affected by life-limiting disease and support their families. At the patients home or other care settings and in our out patient setting, as required. Under the guidance of clinical nurse specialists you will assess patients physical, psychological, spiritual and information needs, plan, implement and evaluate care, provide specialist symptom control advice and emotional support.Main duties of the job

  • Working as a community nurse, following a structured programme. With expected progression from Band 5 to 6 within 12 months dependent on achieving all competencies.
  • Establish effective working relationships with multi-professional community teams, including GPs.
  • Provide advice and support to patients and their families affected. At the patients home or other care settings and in our out patient setting.
  • Work as a member of the Hospices multi-professional team aiming to provide a high-quality, responsive, skilled and compassionate service.
  • Holistically assess patients physical, psychological and spiritual needs to plan, implement and evaluate their care.
  • Develop knowledge/skills in delivering specialist symptom control advice and emotional support.
  • Refer to appropriate services in order to ensure patients have the support they require.
  • Act as the patient or carer advocate by liaising with other colleagues including the GP, hospital teams or within HOSF and sign posting appropriately.
  • Ensure continuity of care through comprehensive, accurate written recording of notes and thorough verbal handovers.
  • Be able to carry out duties according to the philosophy and reputation of the Hospice. She/he shall uphold and enhance the good standing and the reputation of the nursing profession, being accountable for their professional practice, according to the NMC: The Code.
  • Willingness to progress in their professional career.
  • Support the organisations income generation activity.
