Hospital Alcohol Liaison Nurse - Full-Time, Permanent
Are you committed to delivering compassionate, recovery-focused care for those affected by alcohol use? We are seeking a dedicated Hospital Alcohol Liaison Nurse to join our Inclusion Staffordshire STaRS service.
In this full-time, permanent role, you'll provide a comprehensive treatment service to individuals with alcohol use problems, as well as their families and friends. Operating within the framework of the Staffordshire Community Alcohol Team, you'll support individuals on their journey to recovery, improving their quality of life and fostering positive change.
If you have the required qualifications and experience and have passion for making a difference, we want to hear from you!, + Provision of alcohol treatment within the Staffordshire community to Service Users within the locality.
+ Holding a caseload of service users and being responsible for offering a range of interventions to that individual
+ Working to reduce unplanned alcohol related admissions and attendances to the Hospital setting
+ Supporting a service user through their treatment journey from beginning to end
+ Embracing the concept of recovery and demonstrating this in your clinical work
+ Ensuring that your work is evidence based, up to date and in accordance with current policy and legislation.
+ Delivering training and support to Hospital staff
+ Keeping up to date and accurate records and documentation, + Adhere to professional codes of conduct including accountability, confidentiality and commitment to high quality service
+ Maintain security of all records and information whilst adhering to Trust and Partner agencies confidentiality policy
+ Be responsible for ensuring that all service users have a clearly defined and agreed individual programme of care designed to meet their needs, deliver the intervention and ensure discharge of services.
+ Manage a defined caseload of alcohol dependant patients, according to care plans agreed with the service user, carers and responsible Clinician.
+ Provide alcohol interventions and clinical assessment within Staffordshire Hospitals and Emergency Departments
