Hospital at Home Support Worker

The purpose of this role is to provide high-quality support to older people receiving hospital at home support from the Rapid Medical Service Frailty Team. You will have a clear understanding of the needs of older people and will actively promote their wellbeing during their time receiving treatment at home and as they regain their confidence to once again manage their daily activities.

Support will be person-centred and will focus on preventing admission to hospital. You will actively promote the wellbeing of older people and will provide practical and emotional support to allow them to remain living independently. You will also provide advice and guidance and signpost to other relevant services as required to facilitate the needs of older people.
