Full Job Description

We have an exciting opportunity for band 5 staff nurses to join our brand new Hospital at Home service. This is a thrilling chance for a highly motivated staff nurse to join an expanding team and develop autonomous practitioner skills whist caring for our frail patients within the community. The service will be based out of our Frailty Assessment Unit at the acute hospital sites and will work closely with our Acute Frailty Teams to identified patients suitable to be cared for in their own homes. The service will provide 7 days cover and must be able to meet the travel requirements of the role., The post holder will be responsible for delivering a high standard of care to patients and their loved ones in there own home. They will organise and plan care, depending upon the patients needs ensuring all care needs are met. They will be accountable for their care and supported by colleagues. The care given will be in a patients own homes and will require those involved to travel within a geographical area.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
