Housing Support Mentor

Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services - and to prevent them becoming disenfranchised and isolated., This service will provide high quality and effective housing related/floating support service for individuals experiencing substance use issues across the County of Pembrokeshire. The support will focus on an individuals strengths enabling them to feel safe and secure with the long-term aim of them living substance free and independently in their own homes.
Staff are to work with service users in areas such as employment, training and education, debt management and daily living skills to promote wellbeing and a healthy lifestyle.

Staff will be expected to be flexible and work across the hours and days of service delivery and will be part of a rota of on-call service for out of hours provision where necessary.
