Full Job Description

The CNS will work as a fully integrated member of the multi-disciplinary team, within the wider team, caring for the child with hypertension. Within the renal outpatients

Supporting patients in between clinic appointments and being the primary point of access for patients, acting as source of initial specialist advice and resources

Work autonomously in the delivery of hypertension clinics.

Maintain good communication skills enabling maintenance of high-quality service within a complex and challenging environment across primary and secondary care.

Liaise with NHS England and HSS to develop and implement guidelines

Provide effective nursing leadership expertise and support to teams/wards and departments that may require input to provide high quality care to children, young people and their families.

Train and support children, young people and their families to manage their condition/s at home, school etc. as appropriate.

Maintain contact with the patients and families during any hospital admission and support ward staff in providing continuity of care, planning discharge and act as a key liaison for external agencies following discharge.

Applicants should have previous experience at Band 6 level of caring for children and young people with acute and chronic renal disease.
