IBD Clinical Coordinator

An exciting opportunity has arisen to join the small nursing team delivering care to patients with Inflammatory Bowel Disease (IBD: Crohn's disease and ulcerative colitis). This is an interesting and varied post involving face to face as well as remote care, primarily in the coordination of IBD nurse interventions. Your input will be in specialist immunological treatments, managing the home testing programme, surgical pathways, patient information materials and other projects that give additional breadth to the role. You will have support from the IBD Nursing team and, where needed training can be discussed around skills such as venepuncture, cannulation, and following agreed plans of care.,

• Work across departmental boundaries to coordinate with day case, pharmacy, surgery and administration to ensure patients follow appropriate pathways
• Ensure clinical areas are resourced and work alongside the IBD nursing and day case teams to deliver advanced therapies; cannulating, monitoring and taking blood tests
• Coordinate care through key transition points such as surgery, paediatric to adult care, home and hospital and between different treatments
• Manage home testing services; processing referrals, results for faecal home calprotectin and ensuring completeness of communication, · In collaboration with a registered practitioner, use a variety of sources of information to assess the patient's needs and understand the knowledge that underpins this assessment.
· Recognise when a patient's condition changes and seek expertise from registered nurse. · Assess patient's ability to understand information given and how to overcome these barriers. 2. Planning of Care · Work in collaboration with the nurses and follow departmental protocols to formulate an individualised care plan, describing interventions that will achieve realistic goals. · Follow individual plans of care, thus ensuring continuity of care. 3. Delivery · Maintain the dignity and privacy of all patients and respect their right to confidentiality. · Carry out agreed components of planned patient care in collaboration with a registered practitioner, but without the need for close supervision. · Maintain accurate, legible and legally substantive records of care given and observations obtained and recorded. · Ensure that all patients, staff and visitors are treated with equality and respect. · Provide routine practical information and facts about the patient's treatment and access to results using tact, reassurance and empathy. Escalate to nurses when appropriate. 4. Evaluation · In collaboration with a registered practitioner, use a variety of sources of information to evaluate and re-assess patient care. 5. Clinical skills · Carry out a range of patient care tasks in support of registered nurses in by independently performing and documenting clinical observations, cannulating, taking blood and removing patient's cannula when required. · Undertake specific clinical activities, relevant to the clinical area that demonstrates advanced technical and/or caring skills and that requires additional training and assessment as above. Demonstrate a base level of theoretical knowledge behind these practices. · Demonstrate competence in performing and recording clinical observations of patients, and an awareness of their relevance. Alert senior members of staff to abnormal clinical observations in a quick and appropriate manner. · Demonstrate good communication skills when dealing with patients, relatives and members of the multidisciplinary team utilising sensitivity, empathy and the ability to manage conflict. Demonstrate an awareness of the barriers to communication and understanding. · Facilitate assessment programmes such as the faecal calprotectin home kits; liaising between multidisciplinary teams as well as company suppliers as appropriate · Record all care given to patients in an accurate, legible way that meets legal requirements. · Adhere to moving and handling policies and guidelines relating to patients and equipment Organisation: 1. Promote effective communication between all members of the multidisciplinary team. 2. Organise the workload of advanced therapy treatments and other IBD-related treatments for the IBD nursing team working closely with the Day Case Units at the JR and the Horton hospitals. 3. Undertake agreed aspects of clerical work relating to patients and the department such as booking and recording appointments on the electronic diary. 4. Assist in maintaining the safety and cleanliness of the environment and equipment for patients and colleagues. 5. Assist in the orientation and training of new Health Care Assistant (CSWs) act as a mentor and role model to new CSW's. 6. Develop and deliver clear surgical support pathways between those going for surgery and the IBD nursing team. 7. Develop and deliver clear transition pathways between paediatric and adult services especially for those on advanced therapies. Personal: 1. Demonstrate and maintain structured professional and personal knowledge-based development. 2. Discuss and review personal and professional development needs during an annual appraisal and develop, agree and document action points in a personal development plan. 3. Complete a portfolio of probation/beginner clinical competencies and to review progress with unit manager and team leader at regular intervals and through appraisal process 4. Acknowledge personal limitations of competence and do not act outside of these. 5. Ensure mandatory / statutory training is carried out / updated according to guidance., 1. Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through self-education and attendance at relevant courses of study. 2. Contribute to the development of the service and presentation of the service to the multidisciplinary team with the IBD nursing team. 3. Ensure key educational resources are available as needed to the nursing team. 4. Identify educational needs of patients, families, carers and staff involved in the delivery of the IBD service and refer to the IBD nursing team as needed. • + Patients going through transition from paediatric to adult services + Patients with questions about the advanced therapy treatments + Patients wanting to access their results Service Improvement: IBD Clinical Coordinator will; 1. Developing and delivering key pathways alongside the IBD nurse team will be key to this role 2. Assist in the evaluation of the IBD service and patient care, including monitoring of patient experience. 3. Identify areas of risk and poor quality, and address these through appropriate governance structures and forums. 4. Actively participate in projects that develop staff and unit profile. 5. Participate in and support research projects within the area on an adhoc basis. 6. Develop management skills in self and to teach these to junior staff commensurate with their grade. 7. Have responsibility to ensure annual appraisal, agree objectives which reflect local and national service needs and develop personal development plans. 8. Ensure statutory and mandatory training updates, as well as role /speciality updates are completed and necessary records maintained.
  
  Personal attributes you will need are excellent organisational and communication skills. You must also be proactive and confident in discussions with patients and the multidisciplinary team to ensure safe and compassionate patient care. From a practical perspective this job would suit someone who is able to work within school hours each day and spread the 22.5 hours over the Monday - Friday week.,
