Full Job Description
An exciting opportunity has arisen to become part of IBD Specialist Nursing team as a Band 6 support nurse that can offer new experience and learning opportunities in a specialist area. To establish an IBD nursing role that is integrated into the multidisciplinary team to ensure individualised clinical management for patients with inflammatory bowel disease. To work trust wide to ensure continuing collaboration between patients and the professional team in line with National Standards.
- To provide comprehensive clinical skills, information, advice and support to both existing and newly diagnosed patients with IBD.
- Ensure rapid access for patients to specialist care and advice through face to face and telephone contact.
- Work as an independent practitioner both on the ward and in the outpatient setting, including Primary Care, working with your own case load alongside other members of the multidisciplinary team including gastroenterologists, surgeons, dieticians.
- Assess patients prior to their treatment.
- Review and monitor on immunomodulator/biologic medications.
- Participate in clinical audits and research in specialist areas.
- Participate in the education and training of multidisciplinary staff within the Trust.
- Provide a flexible approach to collaborative working to ensure patients have seamless access to specialist care.
