Full Job Description

Are you interested in being a part of our vibrant Crisis team? We are looking for an experienced, registered health care practitioner who is passionate about supporting service users in accessing support from the acute care pathway. The CRHT Discharge Facilitator will support the Trusts Crisis Resolution and Home Treatment Team in managing patient flow from acute inpatient wards into the community by facilitating early discharge from the inpatient units. They will ensure all bed capacity across the Trust is utilised whilst ensuring patients experience safe, high quality care throughout their patient journey. You will work with system partners and colleagues to expedite safe transfers and provide safe alternative pathways for patients. You will work collaboratively with internal, external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised and patient flow is maximised. The role will also support CRHT to manage demand for admission and help support the clinical team to consider alternatives to hospital admission. The In Reach Discharge Facilitator will take an active role in attending 136 assessments to support coordinate ongoing support or admission if required., The post holder will be based within the Crisis Resolution and Home Treatment Team, which is based in the same building as the inpatient wards. You will work closely with both teams, CRHT and inpatients and build up close working relationships with the multidisciplinary team across the acute care pathway. This role will be characterised by a high degree of autonomy and complex decision making.

  • To manage daily referrals from all stakeholders and work along side CRHT to prioritise admissions
  • To work collaboratively with the acute providers by providing In-Reach assessments for patients referred to Solent inpatient units for a period of in-patient admission, across inpatient hospitals
  • Provide and receive complex, sensitive information. Identify barriers to understanding and overcome them in a variety of ways. Be able to communicating in a hostile, antagonistic or highly emotive atmosphere.
  • To lead on service development programmes as identified through analysis, priority plans and feedback
