The team
- Home First provide a short term reablement care and support service to people living in their own homes

£27,711 per annum (pro rata for part time)

Your role and responsibilities
As a key member of our integrated health and social care network, you'll conduct asset-based assessments, provide personalised support, and deliver innovative solutions to maximise independence and wellbeing for our customers. Utilise your expertise in functional assessments, reablement techniques, and digital care solutions to create tailored support plans.
