Full Job Description
The role is to work with your allocated customers to identify and deliver personalised, good quality outcomes to the highest of standards. You will work as part of the Home First team alongside area social work teams as well as the health and voluntary sector to support the effective integration between health and social care.
- You will support the Home First team with customer assessments and reviews
- You will make a positive difference to the residents of Suffolk
- You will receive excellent support and supervision, Step 3 - Upload a supporting statement indicating how you meet each of the criteria in the 'Person Profile' section of the JPP.
- You should use the Supporting Statement template.
- Please use Arial font size 11 and keep your statement to a maximum of two sides of A4.
