As Independent Living Assessor, you can make your own powerful contribution in a team that takes pride in helping people to help themselves. Our Complex Care Services support people with complex health and social care needs to live at home for as long as possible. Joining our close-knit and supportive team, which includes social workers, nurses and occupational therapists, you'll talk to older and disabled residents of Westminster about what could really make a difference in their lives.

It's a varied role, involving a mixture of phone calls, visits, written and computer work. Carrying out reviews of customers' support plans, you'll make sure each plan is sufficient and meets their needs. You'll set goals, coordinate support and work alongside colleagues across Adult Social Care - and our partner agencies - to make sure customers are as independent and safe as possible in their daily lives and activities.

At Westminster you can look forward to the support you need to do an excellent job, with many opportunities for formal training, as well as learning and developing practically in your role. What's more, after two years in the role, you'll have the chance to complete a professional qualification with our support.

An exceptional communicator, you'll bring to the role a deep knowledge of disabilities, disability services, relevant legislation and community support. You know what it means to coordinate care and be a Trusted Assessor within a social/health care environment. You'll highly organised and able to write clear and concise reports. We organise Direct Payments for our residents, so you'll also need to demonstrate good IT and financial skills. Crucially, you are a team player who really cares about making a difference.

Mrs D was frail and unwell. Her son was her main carer, but he wasn't coping. We stepped in to help, but they were hostile to outside agencies. They'd always lived together and were terrified they'd be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son's trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there's nothing more important than living the life you want., Westminster is an amazing place.
