Full Job Description
You will be responsible for managing a wide variety of patient needs through home visits, including but not limited to:
- Chronic illness management
- Respiratory conditions
- Frailty assessments
- Long-term condition reviews
- Post-op recovery care
- Medication reviews
Contract: 6 months to 1 year - Hours: 32hrs per week week days only (flexibility offered)
