Individual Placement Assistant

This roll is pivotal to ensure that we deliver safe, swift discharges from hospital, packages of care to keep customers at home and independent for longer.

Building strong relationships with our providers and colleagues so services and procedures are in place for successful outcomes.

A key part of this roll will be capturing and recording data so that we can shape the future demand for services in the borough.

You will be key to ensuring that invoices are paid on time and in accordance with T&C.

You will have a positive outlook and be able to work as part of a team, embracing change and a fast pace working environment.

You will need to be proactive have excellent attention to detail, organisation skills and be able to plan your own workflow.

You must be able to work under pressure and to deadlines.

Good IT skills are required, being familiar with Teams, Excel, and Egress. You will be open to leaning new systems.
