Full Job Description

We are working in partnership with Portsmouth Hospital University Trust to build a flexible, adaptable, confident, and competent workforce, who can support seamless pathways and improved patient journeys for children between acute or specialist hospital care and community care. As part of this exciting long rotation, we are looking for a well-motivated, enthusiastic individual to join our Integrated Community Children's Nursing service (iCCNS) in Portsmouth.

Our established, passionate, and dedicated Service works collaboratively with children, young people (CYP), families and the wider multi-disciplinary team to ensure safe care is provided within the community setting wherever possible. We deliver high quality, holistic and child centred care to CYP who require nursing intervention, across Portsmouth and the Southeast Hampshire area with acute and long-term conditions, complex needs, life-limiting and life-threatening illness, including CYP requiring palliative and end of life care.

This post will enable you to consolidate your Children's Nursing training whilst further developing skills and knowledge through experiencing various aspects of nursing care to CYP across a range of acute and community settings. The post will offer rotation across, Integrated Childrens Community Nursing Service (HIOWH NHSFT) and the Children's Department (PHT).

This post requires a full UK driving licence and you to have access to the use of a car.

This post will give you the opportunity to complete a rotational post between the Community Team and Portsmouth Trust giving you the opportunity to develop and strengthen skills in both acute and community.

£29,970 to £36,483 a year Please note for part time hours the salary will be pro rata.
