Full Job Description

An exciting opportunity has arisen for a qualified and experienced Senior Nurse to join our Integrated Discharge Service (IDS). We are seeking a highly committed and motivated individual who will ensure that we are providing a patient-focused discharge service to patients across our sites, from a nursing background. The post holder will be an expert in the discharge planning process providing forward-thinking clinical and professional leadership to nursing staff at all levels within the Integrated Discharge Services Team. They will ensure that nursing care is delivered to a high standard and that the nursing resource is managed effectively. Working closely with staff across all disciplines between hospital and community, the post holder will lead the team to ensure safe, timely and effective transfer of patient care on discharge.Main duties of the job The new post holder will work with the Operational Lead of the IDS and with the Head of Operations for Integrated Care. They will lead the IDS team with management and support from five Band 7 discharge specialists covering the different services and boroughs in UCLH. The post holder will also provide senior support to the Admission Avoidance Service in the Emergency Department and Acute Medical Unit. They are also responsible for the management of the Patient Lounge and may also hold their own case load of patients with highly complex discharge requirements. The post holder will work closely with staff across all disciplines between the hospital and the North Central London (NCL) community services to ensure a safe, timely and effective transfer of patient care on discharge via the two UCLH Transfer of Care Hubs.
