Inviting applications to join our Integrated Discharge Team as a Facilitator at Southend Hospital!
We are looking for a new member to the team who is patient focused and is keen to be involved in a dynamic environment where there is a constant drive for quality improvement to positively impact the outcomes of our patients.
As this role evolves, the opportunities to increase knowledge of discharge to assess and MSE working - whilst supporting the safe and effective discharge for patients from our acute trusts.
This role works across a 7 day period including Bank holidays. If you are interested in the role and believe you hold the qualities we require, then apply today!, You will ensure the delivery of high quality, clinically effective and holistic care, through effective collaboration with multidisciplinary colleagues in every day practice.
Assume accountability and responsibility for own decisions.
In conjunction with the IDT Assessors, providing support and training to healthcare assistants as appropriate to their experience.
Presenting a positive role model; striving to continuously improve standards of care whilst keeping up to date with innovations and change.
To contribute to the basic assessment and development of care plans for patients referred to the Integrated Discharge Team
To provide planned advice, support and instruction to patients, external partners including care homes, care agencies and equipment services and where relevant their carers and other professionals, to promote understanding of the aims of their discharge plans to ensure a consistent approach to care and discharge.
Prioritise own case load and provide facilitation to IDT Assessors when and as needed.
To promote the use of the Discharge Lounge to ensure patient flow and experience in discharge planning.
Educate and promote patients and carers to participate effectively in discharge Planning where appropriate.
To gather core information to contribute to discharge plans for patients, including EDD's and MFD dates and feedback to the team. To be competent in completing Assessment and Discharge Notification's in line with the Care Act., We work together, and in conjunction with MSE Health and Care Partnership, to deliver excellent local and specialist services, to improve the health and wellbeing of our patients in a compassionate way, and provide a respectful, but vibrant place for staff to develop, innovate and build careers.
