Full Job Description

Are you a qualified and registered Nurse, Social Worker or Occupational Therapist seeking a new and exciting challenge?, The post holder will use their experience and knowledge of specialist discharge pathways and out of hospital provision to influence discharge decisions; promoting the alternatives to restricted environments and working with the patient and the clinical team to increase confidence in the combined impact of multi-agency discharge support, balancing risk and responsibility. The post is largely ward based supporting the ward Multi-Disciplinary Teams around effective discharge planning. Service need is across a number of our LCFT wards and the post holder may be required to travel across LCFT hospital sites.

You will have a role in contributing to and reviewing the discharge plans of patients to ensure that the patient is progressing (including rehabilitation) to the least restrictive environment that will meet their needs for safety and service quality. You will be responsible for making recommendations for patients who are suitable for transfer to a less restrictive environment and will support the Care Coordinator with this process.
