As part of the Berkshire West Community Respiratory Service, you will providehigh quality,evidence-basedcare for patientsundertakingCardiac orPulmonaryRehabilitation.The roleincludes assessing, planning, implementing,monitoringand evaluatingexercise programmes according to the needs of the referred individuals. They will be based in the community (at leisure centres/community centres or at the client's home) as a lone worker and with other members of the multidisciplinary team.

As a team player,you will be workingwithin theCardiac and Respiratory Specialist Services (CARSS) including theRespiratory Services (and Pulmonary Rehabilitation)Heart Function Service and Cardiac Rehabilitation, all based at Coley Clinic.
