Full Job Description
We have an exciting opportunity to join our West Integrated Transfer of care Hub (iToCH) as one of our administrators to support the hub with processing all the referrals coming into the West community service.
The Administrator will be responsible for reviewing key blockages to patient flow, highlighting key issues to Team Managers maintainingaccurate and timely feedback to referrers, liaising with multi-agencies for the co-ordination of patient flow.You must possess excellent communication and people skills; have the ability to work under pressure, whilst being a committed member of ourteam.
This is a full time position covering the hours of 8am to 8pm 7 days/week, including bank holiday along with the other administrator within the team., The role of the Admin Support Worker is to work with prioritisers who are Nurses, Social Workers, Therapists and other members of staff employed within the multi-professional team, processing and managing the flow of patient referrals through the Transfer of care hub and local point of access.
The Admin Support Worker will monitor the referrals on the electronic referral system (STRATA) on a continual basis, triaging in partnership with the multi professional team. referrers
