Full Job Description

We are looking to appoint a Mental Health Nurse Practitioner/Non-Medical Independent Prescriber (NMP) to join our Integrated Mental Health team based in Lichfield. This role will involve delivering on agreed reviews of prescribed medications for people who are under the Essential Shared Care Agreement (ESCA) with their GPs., The post holder will work closely with their colleagues to deliver on quality and physical health quality improvement and will be responsible for ensuring high quality care for service users within the community. This includes prioritising physical health needs, promoting engagement with screening, assessment and monitoring of physical health and prescribed medications in partnership with primary and secondary care sectors.
Part of this role will also involve working within the IMHT, developing and delivering agreed clinical processes which will include working collaboratively with the clinical team to assess, review and monitor people's health including reviewing prescribed medications as an NMP

The team is looking for an individual who is enthusiastic, passionate, motivated and one who is willing to make a difference to their community, supporting individuals with their mental health and physical health care needs. The successful candidate will be self-motivated and able to work in a fast-paced environment. You will be able to work autonomously and as part of a team. There is expectation to deliver review appointments within primary care facilities and therefore access to suitable transport during work hours is essential.
