Full Job Description
To contribute to the provision of an effective and efficient ISPA service including answering telephone calls from patients, carers, and other health care agencies and professionals and accurately receiving and recording information in order to convey to other staff through the electronic patient record or verbally in a timely way. The administrator will also assist in processing email referrals and messages received by the team. The administrator role will adhere to the service specification and operating procedures, and provide administrative support to multidisciplinary staff within the Integrated Community Teams (ICTs), Frailty Support Team, and elsewhere within the division if required. The ISPA receives referrals from patients, their families and carers, GPs, nurses and other healthcare professionals. There will be a need to allocate to, prioritise, and communicate with community teams to ensure seamless, consistent and safe care to patients., Respond to patients, carers and other professionals in a courteous, professional and effective manner whilst upholding Trust Values
Process telephone calls appropriately
Monitor and process electronic and verbal referrals
Update Electronic Patient Records
Book appointments for daily visits and clinics
Book Patient Transport as directed
Undertake other administrative duties within the ISPA as specified by team lead
Liaise with Team Leaders, Community Matrons and staff within the South West Hampshire Division to provide a safe, effective and timely service to patients and carers
Provide effective administrative support to the Integrated Care Teams
Ensure governance and confidentiality guidelines and requirements are fully adhered to, processing all communications appropriately within guidelines
Provide a flexible and responsive service according to the needs of the organisation to ensure service delivery standards are maintained at all times
Support colleagues within other administration teams
Adhere to HR policies and engage constructively with appraisal and supervision
Promote better patient care at all times and utilise resources as efficiently as possible
