Full Job Description
An opportunity has arisen for STI-2 Junior clinical fellow level doctor within our Frailty services at Royal Lancaster Infirmary working as part of our Frailty SEDC and elderly care wards.
The Frailty SDEC pathway works on a "early intervention, better outcomes" philosophy with a focus to returning patients to their usual place of residence on the same day as presenting in the Emergency Department where appropriate.
You will work as part of a multidisciplinary team that currently consists of a Consultant Geriatricians, Advanced Nurse Practitioners, Pharmacy Technicians, Dieticians, Occupational Therapists, Trainee ACP and REACT Community Assessors who provide a 7 day service assessing Frail & Older patients attending the emergency department.
This service has built strong links with our community services and establish formal links with pre-hospital urgent and emergency care services to offer direct referrals.
Part of this role will be to support our elderly care wards and Acute Frailty unit as needed.
This is an exciting time for our services and we hope you will want to play a part in developing a gold standard service which will achieve our aim of delivering excellent care in the Morecambe Bay area for our Frail and older patients., As part of a skilled multidisciplinary team, to be responsible for undertaking patient assessments for Frail & Older patients attending the emergency department & Frailty Same Day Emergency Care area to help patients return to their normal place of residency at the earliest opportunity. This means ensuring that full documentation, including TTOs, discharge letters to other specialties/ GPs etc. are accurate, comprehensive and timeously completed and ensuring investigations, follow ups and referrals are arranged where required.
To ensure the Frailty SDEC provides an effective service 7 days a week, this role will require weekend working when the rota is fully established on a 1 in 4 rotation.
Part of this role will also be to support our elderly care wards of which there are 3 and an Acute Frailty unit.
The appointment will be 40 hours per week.
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
