Full Job Description

  • Provide dietetic assessment, advice, and care planning to community patients.

  • Collaborate with other health professionals to deliver a multi-disciplinary approach to the care of vulnerable elderly patients with nutritional needs.

  • Work with HEF (Home Enteral Feeding) patients to ensure their care is safe and effective.

  • Maintain accurate and up-to-date records of patient assessments, care plans, and interventions.

  • Ensure compliance with relevant legislation, policies, and procedures.

  • Participate in supervision sessions, team meetings, and training opportunities to support professional development.

    A relevant dietetics qualification.

  • Registration with the Health and Care Professions Council (HCPC).

  • Experience in community dietetics, particularly with elderly patients and HEF.

  • Strong assessment and care planning skills.

  • Excellent communication and interpersonal abilities.

  • Ability to work effectively as part of a multi-disciplinary team.

  • Commitment to maintaining high standards of practice and professional development.

    Pay Rate: Up to £26 per hour (depending on experience)

  • Work Hours: Up to 4 days a week (7.5 hours per day, negotiable days)
