Full Job Description
- Provide dietetic assessment, advice, and care planning to community patients.
- Collaborate with other health professionals to deliver a multi-disciplinary approach to the care of vulnerable elderly patients with nutritional needs.
- Work with HEF (Home Enteral Feeding) patients to ensure their care is safe and effective.
- Maintain accurate and up-to-date records of patient assessments, care plans, and interventions.
- Ensure compliance with relevant legislation, policies, and procedures.
- Participate in supervision sessions, team meetings, and training opportunities to support professional development.
A relevant dietetics qualification. - Registration with the Health and Care Professions Council (HCPC).
- Experience in community dietetics, particularly with elderly patients and HEF.
- Strong assessment and care planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively as part of a multi-disciplinary team.
- Commitment to maintaining high standards of practice and professional development.
Pay Rate: Up to £26 per hour (depending on experience) - Competitive pay rate of up to £26 per hour (depending on experience).
- Flexible working hours (up to 4 days a week, 7.5 hours per day, negotiable days).
