- Conduct Occupational Assessments and Interventions, emphasizing relapse prevention, establishing roles and routines and promoting recovery.
- Play a pivotal role in Discharge Planning by conducting Assessments, coordinating Interventions and ensuring a smooth transition for Patients back into the community.
- Conduct O.T Assessments in home environments to identify O.T interventions and Equipment needs as part of Discharge Planning.
- Work with Larch Ward's forthcoming Home First service staffed with O.T and O.T Clinical Support Worker. The Home First service provides up to 6 weeks OT intervention to support earlier and/or safer discharge from the Ward. What We Offer:
