Full Job Description

You will have medical responsibility for patients in CDAT & NIRRT. You will conduct full psychiatric assessments either at the team base or in patients' homes. Following an assessment, you will formulate management plans. You will carry out risk assessments on all new cases and ensure risks are addressed in management plans. You will record clinical information, including all diagnoses, medication, risk assessments, DIALOG+ rating scales and DIALOG+ care plans and outcome all appointments using the Trust's electronic patient records system RIO, and update patients' records regularly. You will discuss your assessment and management plan with your consultant. You will send the GP and client a copy of your assessment within a week of assessment.
There are weekly team meetings, and you will take an active role within the Multidisciplinary Team at all stages of managing patients. This includes urgent assessments, joint reviews with team members and CPA reviews. Much of the work is multi-disciplinary, so you will have the opportunity to work with all members of the team. You will dictate or type letters to GPs after each patient contact or change in management plan to keep them informed and record changes in management on RIO.

In order to meet the needs of the new Partnership services you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Enfield and Harringay Trust and Camden and Islington Trust. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.
