LCW Advanced Practitioner

A new opportunity has arisen for the Advanced Practitioner to join our dynamic team.The post holder will be an experienced clinician who, acting within their professional boundaries, will work flexibly to provide care for patients from initial history taking, clinical assessment, diagnosis, treatment to evaluation of care. The clinician in this role will be key to supporting clinical work in the Integrated Urgent Care Service, whilst acting as a positive and enthusiastic role model, supporting junior staff and developing our workforce., Clinical Practice Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undiagnosed and undifferentiated illness Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly Telephone triage, calls from external clinicians, patients and others into a range of services appropriately as required Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnosis and manage treatment plans for patients at risk of developing a long-term condition or deteriorating outside of the hospital setting (as appropriate) Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan according to competency, whilst seeking opportunities to develop knowledge around medication prescribing and administration where appropriate.
