Full Job Description

The post holder will be responsible for the assessment of clients with Learning Disabilities in any setting (i.e. home, acute settings, residential settings) to determine eligibility to receive Fully Funded NHS Continuing Healthcare [CHC] or Funded Nursing Care [FNC]. The post holder will act as senior nurse assessor/co-ordinator of a Multidisciplinary Team to establish eligibility and working with the client and their family, identify a suitable care package. They will ensure the review process of all Funded Nursing Care/Continuing Healthcare is undertaken in accordance with the National Framework Guidance for Continuing Healthcare (revised 2018)., The post holder will ensure that assessments and reviews are carried out in a timely manner according to the local KPIs in order to meet the national performance targets. The post holder will form part of the senior nurse assessor team for the CHC team, ensuring that clients receive high quality care and provide specialist advice regarding Learning Disabilities to the CHC team. The post holder will also lead on ensuring all clients with a Learning Disability who have care delivered through a Personal Health Budget, have appropriate care and support plans in place, and will support in the management of the PHB, resolving issues as they arise.,
