Full Job Description

We have an exciting and rare opportunity to join and lead our Advanced Clinical Practitioner team within our Community Health Services. Working and reporting to the Head of Service - Advanced Clinical Practice for the Community Hospitals, the post holder will work as the clinical and operational lead of a dynamic geographically based team in delivering a safe and effective staffing service to support the Advanced Clinical Practice team for Community Hospitals. The post holder will be responsible for the development and operational management of the service reporting to the Head of Service for Advanced Practice. The community hospitals are adult community bed based services with 250 inpatient beds across 13 wards providing sub-acute step down and rehabilitation services across 9 sub-acute wards, 2 stroke rehabilitation wards and 2 wards delivering discharge to assess pathway beds provided from seven community hospitals across Leicester, Leicestershire and Rutland.,

  • Working closely with the Head of Service for Advanced Practice to provide clinical and operational management for the medical and advanced practice service in Community Inpatient wards.
  • Leading a team across one of the two geographical locations within Community Hospitals for Leicester, Leicestershire & Rutland (West Lead Hinckley, Coalville, Loughborough and East & City Lead Evington Centre, Market Harborough, Rutland and Melton).
  • A workplan of 60% clinical; 40% leadership/education/audit and research to reflect the 4 pillars of advanced clinical practice.
  • Working closely with the Responsible Clinician, Consultant and Ward team.
  • Lead, participate in and cooperate with concerns, complaints and investigations as required, providing critical analysis and professional review and supporting investigators within the directorate as necessary.
  • Leadership, planning and delivery of staffing including rostering, recruitment and deployment.
  • Ensuring that teamwork and staff management is undertaken in line with Trust values.
  • To participate in the CHS silver oncall rota., patterns in line with our flexible working policy. For all substantive roles, new staff (excluding medical staff) are appointed subject to a 6-month probationary period (see Probation Policy). All jobs will require permission to work in the UK.
