Full Job Description

Would you like to be the Lead Nurse for the Cardiac Nurse Team with responsibility for delivering cardiac rehabilitation to patients from the Harrogate and Rural District in both the hospital and community as part of the integrated cardiac rehabilitation team?
You'll be a part of a small and dynamic team, providing support and education to patients admitted with heart failure to Harrogate District Hospital and providing support and education to patients admitted with arrhythmia to Harrogate District Hospital., The cardiac nurse team is responsible for delivering the full programme of cardiac rehabilitation from admission to discharge at approximately 3 months both in the hospital and in the community for patients following heart attack, heart surgery or any relevant condition.
The nursing team is responsible for providing clinic follow up to patients who have undergone angioplasty. You will be responsible for seeking out and seeing patients admitted with heart failure for advice and education whilst an inpatient and for organising community follow up with the communities Heart Failure teams as appropriate.
You will be responsible for seeing inpatients admitted with arrhythmia to provide support and education and contact details for support after discharge. The team is also responsible for entering data onto the MINAP, NACR and National Heart Failure Audits.
