Full Job Description

An exciting opportunity has arisen for a full time Lead Care Coordinator to join our Integrated team within Derby City.

We are looking for a highly motivated and enthusiastic individual to provide leadership and directly line manage a team of care coordinators.

To act as a lead co-ordinator for the Care Coordinator Team working across GP practices in the locality. The post holder will achieve this by timely communication with team members; co-ordination of service delivery and operational meetings.

To be a role model to the team and managerial support to the Integrated team lead.

Monitor performance and attendance and supervise direct reports.
To maintain IT based information systems and take responsibility for the production of relevant performance data.

Key duties will include caseload supervision, appraisals performance and attendance management, recruitment, leading team and service meetings, supporting service development, liaison with service partners and reporting contractual change

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

During the pandemic response, we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
