Full Job Description

We are looking for an exceptional person to provide organisational leadership in medicines optimisation for people with learning disabilities with/without autism (PwLDA). A key priority within this new and innovative post is to lead the 'Call to Action' to stop the over-medication of people with a learning disability (STOMP-STAMP), for which the post-holder will be the nominated lead. This role will provide a dynamic link between person-centred care and collaboration, professional practice, leadership and management, education and research (the RPS 'four pillars of practice) by ensuring PwLDA get the best experience and outcomes from their medicines, whenever and wherever their care is delivered across the Northeast and North Cumbria area.

This post has been commissioned by the NENC ICB and we expect the successful applicant to work towards credentialing as a consultant ready pharmacist.

Working to optimise patient outcomes from their medicines within LDA across the trust and to link in with primary care colleagues in supporting this programme.

Leading on the STOMP (Stopping Overmedication of People with Learning Disabilities and Autism) and STAMP (Supporting Treatment and Appropriate Medication in Paediatrics) programme across the trust and to link in with primary care colleagues in supporting this programme

Providing clinical leadership, management, support and guidance to clinicians across the different pathways in supporting STOMP and STAMP

Main duties of the job

Working as an independent prescriber and team member within defined clinical services across CNTW

Collaborating with members of the nursing team in order to improve the quality, safety and cost effectiveness of medicines use and physical health monitoring of patients with a learning disability diagnosis

Contributing to the development of mental health prescribing policy across the region in line with STOMP-STAMP guidance, collaborating with GPs, primary care pharmacists, secondary care specialists and other healthcare professionals

Ensuring systems and processes are in place for training and supervising staff undertaking enhanced STOMP-STAMP medication reviews to ensure they are suitably trained and competent

Leading on and providing subject expert input into associated groups regarding clinical pharmacy and medicines optimisation

Delivering education, training and clinical supervision in STOMP-STAMP across the trust and sharing learning across the NENC ICB

Contributing to the evidence base on the effective use of medicines and medicines optimisation in people with a learning disability, autism or both and to lead the research agenda relating to STOMP-STAMP in the trust collaborating with stakeholders and clinical colleagues

Working to improve health outcomes for PwLDA using a holistic approach to care, ensuring the safe and effective use of medicines

Progressing towards Consultant Pharmacist credentialing through the Royal Pharmaceutical Society (RPS). There will be an expectation that this will be completed within a time frame agreed between all parties on commencement of this role.
