We have an opportunity for a Lead general practice nurse:

The Nurse led model enables team members to develop the full range of Primary Care skills and expertise required to flourish within this growing and challenging area.

As Part of Sandwell and West Birmingham NHS trust we are able to offer full AFC terms and conditions.

You will have accountability for delivery of services in line with primary care contracts such as QOF, PCCF and the IIF .

Working with the PCN head of nursing and along side other Lead nurses the role requires a committed and strategic thinker to provide leadership support and management to the nursing team.

Main duties of the job

To ensure the safe , effective and smooth running of the designated outlet in terms of line management of the nursing team.

To deliver high quality long term disease management.

Over see the training and development of staff within the team.

Be responsible for infection control within the outlet.

You will have responsibility for line managing the outlet nursing team.

You will contribute towards the PCN nursing strategy and delivery of services.

You will be working as lead in a specific chronic disease ensuring the highest quality service and care to our population.

The role also requires visiting our housebound population and traveling between sites there for a full driving license and access to a car for work is a requirement.
