Full Job Description
The Nurse Specialist/Team Leader will be the clinical expert and professional resource to the Multi-Professional team, patients and carers within their Speciality Service.
The post holder will take overall responsibility to develop the specialist area of practice in partnership with other members of the speciality team (General Manager, Lead Clinician, Matron, Service Manager etc) will work collaboratively to develop services and quality of care delivered.
The post holder will manage and lead the whole service team comprising of specialist nurses/advance nurse specialists and other team members and in addition lead the development and delivery of a programme of practice and professional development for staff within the speciality. This will be achieved through working in partnership with lead clinicians, managers and matrons., Managing/Co-ordinating nurse led clinics.
Working in outpatients clinics
Acting as a resource for specialist knowledge and advice
Patient communication, education, information
MDT meetings, liaising with GPs, district nurses
Providing specialist, knowledge, skills and expertise in the practice area, ward rounds
Initiating, developing flexible and innovative approaches to meet patient needs.
Attend weekly 6:4:2 MDT Cath Lab meetings.
Take an active role in the development of/ updating relevant guidelines and documentation regarding the Cardiac Cath Lab.
Keep up to date with the Trust access policy and ensure patients meet the 18 week pathway targets. Working with the management team to implement strategies to tackle and reduce breaches.
Liaise with the ward nursing and medical teams to ensure appropriate referral and preparation of patients prior to their procedure.
Be flexible to an ever changing environment with regards to Cardiac Cath Lab activity.
