Full Job Description

We are looking for an innovative, skilled, compassionate Learning Disability Nurse for our new Specialist Community Learning Disability Service as Lead Nurse.

This is an exciting opportunity and your expertise will be essential in supporting the changes in the nursing team and providing leadership for the nurses, as well as being part of the LD leadership team.

The service offers standard and enhanced pathways and will be more responsive and accessible, providing specialist health assessments, support, and care for adults aged 18 and over. Positive Behavioural Support underpins person centred care, in line with National agendas. Care will be provided in a community setting, close to people's homes. A specialist evidence-based person-centred model is used within the nursing team.

We support our staff to do the best job they can and work alongside service users, their families. The transformation has been co-designed and co-produced with stakeholders bothinternal and external to our organisation, alongside experts by experience, carers and service users., As a key member of the Clinical Leadership Team for the service, working with support and direction from the Head of Nursing, the lead nurse will work as part of this team to implement the agreed clinical model for the learning disability service and support the professional leadership of nurses within their service line.

As the Lead Nurse you will be highly visible and accessible to staff in the service, leading by example in empowering and developing staff to work in coproduction with service users, carers and community groups to deliver high quality, evidence-based care.

You will be hold a small caseload of clinical work with service users and carers which will account for 40% of your time, ensuring that you remain clinically active, relevant and connected to real time practice.

To act as a specialist nursing advisor in the field of learning disabilities promoting the strengths and contribution of people with learning disabilities. Ensuring contributions to service development, continuous improvement, evaluations and strategic thinking.

Provide professional clinical leadership to the nursing staff group as required by the service including recruitment, supervision, and development of these staff within their area.

Provide direct line management and professional supervision to nurses within the community team.
