Full Job Description

Haringey GP Federation (HGPF) has set up a service to identify long term conditions in patients in areas of deprivation who may experience healthcare inequalities. These patients will be invited for a health check with a view of identifying and managing Hypertension (HTN), Chronic Kidney Disease (CKD) and Chronic Obstructive Pulmonary Disease (COPD). As a part of this role you may be asked to support wider nursing work, including supporting other respiratory services, or nursing within the integrated services teams or at practice level.
