We are looking for clinicians who have a specific interest and passion for the care of older adults.
The team provides assessment, treatment and care for patients who experience organic and/or functional illnesses, as well as support to their carers.
This role involves holding a caseload of people with a mental health condition (functional or organic), assessment and intervention work, alongside other clinical members of the team. The post holder will have opportunity to develop leadership skills and to have a role within the team of a link worker or champion (eg. student placement link worker, medicines champion, clozapine link worker).
Within the team, the Lead Nurse will work with other Nurses, NMPs, Occupational Therapists, Psychologists, Nursing and Psychology Assistants, Consultant Psychiatrists, Junior Doctors and Administrators to provide the best care for the population we support., The role of Lead Nurse can be challenging and varied with the management of a complex caseload, completion of initial assessments, undertaking the role of duty worker, attending CST meetings and being part of the team's Care Homes Project., Join 'Team Derbyshire Healthcare' and become part of a talented, compassionate and enthusiastic workforce committed to a vision of 'making a positive difference in people's lives'.
CQC rated us as 'GOOD' overall, commenting on how our colleagues "treated patients with compassion and kindness" and "felt positive and proud about working for the trust.", The successful candidates will receive clinical and management supervision on a regular basis, and will be supported to develop as a leader personally and professionally.
+ Carrying a complex caseload.
+ Assessing service users with a functional/ organic illness and formulating care plans in the diverse area on Derby City.
+ Undertaking duty; triaging referrals, communicating with other disciplines and signposting to the appropriate services.
+ Supervising staff.
+ Being actively involved with the care homes project and CST meetings.
+ Working within a multi-disciplinary setting.

The successful candidates will need excellent communication skills, the ability to complete thorough assessments, including safety assessments, be able to produce person-centred care plans with a living well/recovery focus and work within the CPA framework.
